Cindy Radavich

Family Nurse Practitioner

I was born and raised in Coeur d'Alene Idaho, but have traveled extensively and lived in Italy for multiple years. Consequently, I have first hand experience with the failures of socialized medicine and our current medical system driven by the pharmaceutical and insurance industries. As a provider I  strive to provide personal and wholesome medical care aimed at the individual.                                                                                                                                                    My education includes a Bachelor of Arts in Communication Studies, a Masters of Science in Agricultural Education, a Bachelor of Science in Nursing, and a Masters of Science in Nursing in Family Medicine. I am licensed in the state of Idaho as a Registered Nurse (RN) and as a Family Nurse Practitioner (FNP), and have worked in wound care with both chronic and acute patients as well as in multiple areas of acute hospital care. Previously I was licensed as a Hearing Instrument Specialist for 17 years and am currently a certified Stop the Bleed instructor.                                                            My faith in Christ and commitment to my family are central to my life. I've been married over 30 years to my husband Jay, raised nine kids, homeschooled for 25 years and help run a small family farm. When free time allows I enjoy painting, gardening, hiking and exploring .

Philosophy of Care                                                                                                                                 As a Family Nurse Practitioner, my focus is personalized preventative family care for patients of all ages. I am committed to provide you with the best medicine offers through both traditional and alternative methods.  My goal is to create an individualized plan of care for each patient and to identify issues and potential risks before they become life altering medical problems. If medical problems are already present, my goal is to minimize and control them with healthy immune supporting options rather than pharmaceuticals whenever possible. This can be very challenging, but I am committed to thorough and extensive research to find solutions for my patients.  Consequently, getting to know my patients on more than a superficial level is important. I need to know and understand you and your life, therefore initial and annual appointments tend to take approximately an hour or more. While there may be times that short appointments are appropriate, I do not schedule overlapping appointments.  And, I do make house calls when its appropriate.


Services and Pricing Plans

Plan A

                       Individual inclusive                                                      $75 per month                               includes all individual visits

Annual commitment for adults, with auto-payments. These fees include  ANY  appointments needed for your healthcare that can be addressed in  office or by telehealth appointments.

For families: first 2 children are an additional $25.00 each, the 3rd & 4th child are an additional $10.00  each.   All other children in the family are added to plan at no additional cost.

One-time $100 enrollment fee per adult

▪ Child Enrollment only (no parent on plan) $50 per child per month

  • INCENTIVE: Pay for full year in advance, enrollment fees are waived

This plan is a one-year non-refundable contract

PLAN 'A' FEES COVER:                                         -All office visits /telehealth .                              -Regular access to ask questions without waiting to schedule appointments.                 -Initial Basic labs                                                       -Basic EKG                                                                   -Basic Pulmonary function testing               -Minor procedures – such as skin biopsies, sutures, etc.                                              -Deeply discounted in office treatments (ie. labs, respiratory treatments, IV’s etc.) see Service Fee Slip for pricing                               

NOT COVERED UNDER PLAN A:                  -Recommended Supplements/Pharmaceuticals/ Prescriptions.                                                              -Labs beyond basic initial set, although discounts apply                                                          -Pathology fees for pap smears, skin biopsies, etc (discounts applied when possible)                                                                         -Outside testing, procedures                           -Visits/referrals with specialists or therapies, hospitalizations, or emergency room visits                                          -Durable medical equipment

Plan B

Family/Individual Maintenance $30/month + discounted visits

Annual commitment of $30 per month per family, with auto-payments. These fees provide access to discounted  visits for your healthcare in office or by telehealth appointments.

Office Visit $125 (regularly $250), Telehealth $75, visit type is to be determined by provider.

▪ *This plan includes discounted prices of in office treatments (ie. labs, respiratory treatments, IV’s  etc.), See Service Fee Slip for pricing.

This plan is a one-year nonrefundable contract

▪ *Optional upgrade to Plan A as needed by individual patients.      

*Optional 30 day upgrade                      Within 30 days of your Plan B visit you may upgrade to Plan A.

If upgrade from Plan B to Plan A is chosen, 3 months of fees and enrollment fee will be credited. Membership enrollment date will be the date of your initial visit.

Non- Plan Services

    (for non members)

Comprehensive Physicals include :

  • labs,  two visits, one for testing, and one for discussion of all the findings and a plan to address any identified issues.

  • If there is need beyond one follow-up phone call an additional billable Acute Care Visit should be  scheduled.

  • *Optional upgrade Within 30 days of your comprehensive physical you may upgrade to Plan A or B. (Membership enrollment date will be the date of your initial visit.)

      Acute Care Visits will also have:

  • Additional cost of in-house services such as labs, rapid tests, and procedures applied to bill at time of service.

  • One  optional follow-up call related to the visit is included. If your healthcare needs continue, an additional billable appointment will need to be scheduled.

  • *Optional 7-day upgrade to plan A or B. Within 7 days of your one-time visit, you have the option of upgrading to Plan A or B Membership.                                  Membership enrollment date will be the date of your initial one-time visit.
